OnPatient Guide

Once your initial appointment is scheduled you will receive an invitation to OnPatient via email. OnPatient is our patient portal where you will find the paperwork to fill out prior to your appointment which includes—contact information, health history, insurance information, and consent forms. OnPatient can also be used to manage your appointments and communicate with your provider.

Use this guide to help navigate the OnPatient sign up process.  

Once signed in, it will direct you to the page where you can see your upcoming appointment. Click Check-in.

After checking in, it will direct you to the page where you need to fill-out/update all the information needed.

After that, you will see all the forms that need signature at the bottom of the page.

Click on every form to view it and after that, click on I've read this document.

Once you click on that, you will see a space where you can put on your signature. Sign on it and click on Save Signature.

After you finished reading and signing all the forms, your page should look like this:

Make sure it checks all the boxes.

After that, click I'm Done at the bottom of the page.

And this will take you back to the details of your upcoming appointment, and that means you're all set!
